Extreme is one of the five difficulty options in Granny. It was added to Granny in Version 1.3, and it is currently the hardest mode in the game, as it changes various gameplay aspects of the game.

Info
- Granny And Spider Mom are extremely fast (faster than the Player).
- Creaking floors are no longer avoidable.
- In the Basement, there is now iron bars to the secret area tunnel, making it unavailable.
- Though, the Dirt Room is still available for player to go there.
- Tranquilizer Darts, Sauna, Freeze Trap and dashing with the Car will knock her out for 15 seconds.
- Shotgun and Gasoline Can will knock her out for 30 seconds.
- 5 Locks on the Main Door as well as Extra locks: the Plank Lock, Wire Lock, Regular Padlock, Numeric Padlock, Main Door Lock, and the Battery Lock, Switch Lock.
- There is no opportunity to complete the Painting as well as in Hard, therefore ruining the Player's chance to get the bonus day. The reason is because painting is not put in basement.
Trivia
- As of Version 1.8.2, this is the hardest difficulty option in Granny.
- In Version 1.3, Extreme Mode used to be easier, as the Creaking Floors could be avoided and there was no Iron Bars covering the shortcut to the Basement.
- Prior to Version 1.3.2, the only way to get extra locks on was to play this game mode, but now you can even have extra locks on Practice Mode.
- In Version 1.3.2, Extreme Mode is little harder due to unavoidable Creaking Floors and Iron Bars covering shortcut to the basement which blocks the basement side but still player can enter through other side.
- Prior to Version 1.5, Granny would be knocked out for 15 seconds with all weapons, including the Shotgun.
